Minnesota Department of Health Special Instructions

Federally certified Skilled Nursing Facilities/Nursing Facilities (SNF/NFs), Skilled Nursing Facilities (SNFs), Nursing Facilities (NFs) and hospitals with swing beds, all have additional federal reporting requirements. This report is required within 24 hours after discovery of incident and must be filed online with MDH.
