Purpose: To announce the provider attestation toolkit and available training for foster care, DTH, prevocational, structured day and supported living services providers
DHS added provider attestation toolkits for adult foster care (EW/AC), adult/child foster care (CAC, CADI, BI), supported living services, day training and habilitation, prevocational services and structured day services to the Transition plan for home and community-based settings webpage. Please click on “provider attestation” to find the toolkit information.
The attestation process is required for providers of specific HCBS waiver services to demonstrate that they meet new federal requirements or to indicate that they need more time or assistance. Providers must complete this process and comply with all new setting guidelines by Sept. 1, 2017, unless they request an extension during the attestation process.
DHS hosted an HCBS waiver provider attestation webinar on April 11, 2017. Audio playback and the presentation handout can be found on the Transition plan for home and community-based settings webpage under “more information about the provider attestation requirements and process.”
A repeat webinar is scheduled for April 19 from 1:30 to 3:30 p.m. You must have a unique key to register for this webinar.
