Conversion Access for Alternative Care

Definition

AC conversion case management: Support of a discharge planning process to help people relocate to community-based settings.

Eligibility

This service is available to people who:

  • • Reside in a qualified setting*
  • • Will relocate to the community and
  • • Will receive services through the Alternative Care Program.
  • * Qualified settings include:
  • • Certified boarding care home
  • • Hospital
  • • Intermediate care facility
  • • Nursing facility.
  • Access to this service is limited to 180 consecutive days. The 180-day limit is a “per admission” limit. People may receive another 180 days of service if they are readmitted to a qualified setting.

    Covered services

    Activities include but are not limited to the following:

    1. Develop and implement a relocation plan.
    2. Coordinate referrals and help people access services.
    3. Coordinate and monitor the overall implementation of a relocation plan.
    4. Coordinate efforts with the discharge planner and others.
