Definition

Moving Home Minnesota services: Services available under the Moving Home Minnesota demonstration to promote independent living.

Demonstration services

The following services are available through the demonstration:

  • • Caregiver education (Family Memory Care)
  • • Case consultation and collaboration (Pre-discharge and post-discharge)
  • • Certified peer specialist services
  • • Comprehensive community support services
  • • Durable medical equipment and assistive technology
  • • Environmental modifications
  • • Membership fees
  • • Moving Home Minnesota Case Management Services
  • • Overnight Assistance
  • • Non-medical transportation to find housing and/or employment
  • • Personal emergency response systems
  • • Psychoeducation services
  • • Respite services
  • • Supported employment services
  • • Tools, clothing and equipment necessary for employment
  • • Transition planning and transition coordination services
  • State Plan

    Certain children’s mental health state plan services are available with enhanced federal match to the demonstration:

  • • Children’s Therapeutic Services and Supports
  • • Day treatment
  • • Individual, family or group psychotherapy
  • • Individual, family or group skills training
  • • Behavioral aide and direction of behavioral aide
  • • Mobile crisis response and stabilization
  • • Youth Assertive Community Treatment
  • Certain adult mental health state plan services are available with enhanced federal match to the demonstration:

  • • Adult rehabilitative mental health services (ARMHS)
  • • Assertive community treatment (ACT)
  • • Crisis response and stabilization services
  • • Adult mental health targeted case management
  • State Plan home care services (e.g. personal care assistance, private duty nursing, skilled nursing visits, home health aide, etc.) are available with enhanced federal match to the demonstration.

    Note: Any state plan services are available to the participant who has an assessed need.

    HCBS Waivers

    The following Minnesota waivers are available through Medical Assistance at enhanced federal match to the demonstration:
