Downsizing

Definition

Downsizing: Permanent reduction in the licensed capacity of an ICF/DD.

County Support

The host county in which the ICF/DD is located plays the lead role in the downsizing process. Counties review current Local System Needs Plans to determine if the county supports the downsizing of the ICF/DD and decertification of the ICF/DD beds.

Counties base decision to support or not to support downsizing of the ICF/DD on the following:

  • • Ability of the counties to support persons moving from the ICF/DD
  • • Availability of ICF/DD entitlement options
  • • Choice of the persons residing in the ICF/DD requesting downsizing
  • • Regional capacity
  • Facility Planning

    ICFs need to work in cooperation with the host county and the county of financial responsibility case managers to support the identified choices of each person currently living in the ICF/DD.

    Documentation

    If host counties support the downsizing, submit the following to DHS.

    Host counties retain rights to keep ICF/DD bed allocations and move them to a different ICF/DD.

    DHS Approval

    DHS staff review documentation received. DHS bases decisions on the following:

  • • Alternatives that are available should the facility downsize
  • • Choices of persons residing in the ICF/DD
  • • Consideration of the need for beds in the region of the state
  • • Licensing and certification status of the ICF/DD
  • DHS approves the request, denies the request or asks for additional information. Once DHS determines a final decision, DHS notifies host counties in writing. Upon DHS approval, host counties provide copies of the plan to ICFs/DD. Counties may then move forward with plans as written and approved by DHS.

    Submit changes and/or revisions to approved plans to DHS. This includes changes in approved dates or timelines. DHS must approve or disapprove all changes. Only then can counties and ICFs implement changes.

    DHS notifies the Department of Health and DHS Licensing about bed decertifications and downsizing activities.
