Dear Mr. Elliott:
CORRECTION ORDER
An investigation of Bridges MN was completed regarding report number 202206002 which alleged violation of Minnesota Statutes, section 626.557, and Minnesota Statutes, Chapter 245D. This maltreatment allegation concerned neglect. The disposition of the investigation report is false. Two violations were noted.
VIOLATIONS AND CORRECTION ORDERS
The following violations of state and (or) federal laws and rules were observed. Corrective action for each violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violations within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07.
1. Citation: Minnesota Statute 245D.05, subdivision 1, paragraph (a).
Violation: The license holder did not ensure services were provided in a manner that met a person’s health service needs that were assigned in the person’s support plan and/or support plan addendum.
A consumer’s (C’s) support plan addendum stated that staff persons were responsible for scheduling the C’s medical appointments and transporting the C to the appointments. However, between June 2021 and March 2022, the C’s hormone shot, which was supposed to be administered at a clinic every three months, was not completed.
Corrective Action Ordered: You are currently operating pending the appeal of an Order of License Revocation. Because the C is no longer receiving services in your program, you are not required to provide a copy of this order to the C, the C’s legal representative, or the C’s case manager. While you continue to operate, you must maintain compliance with this licensing requirement on a continuing basis.
Repeat Violation: The license holder was cited for a similar violation in correction orders dated September 13, 2018; June 14, 2019; December 10 and 27, 2019; February 5 and 25, 2021; in an Order of Conditional license dated March 10, 2020; and in an Order of Revocation dated June 27, 2022.
2. Citation: Minnesota Statute 245D.05, subdivision 1, paragraph (b), clause (2).
Violation: The license holder did not ensure the C’s health conditions were monitored according to written instructions from a licensed health professional.
On August 2021, the C’s physician ordered that the C schedule an appointment for a biopsy; however, this appointment was not scheduled and/or the biopsy was not completed until July 2022.
Corrective Action Ordered: You are currently operating pending the appeal of an Order of License Revocation. Because the C is no longer receiving services in your program, you are not required to provide a copy of this order to the C, the C’s legal representative, or the C’s case manager. While you continue to operate, you must maintain compliance with this licensing requirement on a continuing basis.

If you believe any of the citations are in error, you may ask the Commissioner of Human Services to reconsider the parts of the correction order that you believe to be in error. If you choose to exercise this right, your request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this letter.
