CORRECTION ORDER
Dear Blake Elliott:
A licensing complaint investigation of Bridges MN, located at 1932 University Avenue West, Saint Paul, was conducted to determine compliance with state and federal laws and rules governing the provision of home and community-based services to persons with disabilities and age 65 and older under Minnesota Statutes, Chapter 245D. As a result of the licensing complaint investigation(s) a Correction Order is being issued.
A. Reason for Correction Order
Pursuant to Minnesota Statutes, section 245A.06, if the Commissioner of the Department of Human Services (DHS) finds that the license holder has failed to comply with an applicable law or rule and this failure does not imminently endanger the health, safety, or rights of the persons served by the program, the Commissioner may issue a Correction Order to the license holder.
The following violation(s) of state or federal laws and rules were determined as a result of the licensing review. Corrective action for each violation is required by Minnesota Statutes, section 245A.06 and is hereby ordered by the Commissioner of Human Services.
1. Citation: Minnesota Statutes, section 245D.07, subdivision 1a.
Violation: For one person whose record was reviewed (P1), the license holder did not provide services in response the person’s identified needs, interests, preferences, and desired outcomes as specified in the coordinated service and support plan and support plan addendum (CSSPA) as required. The license holder failed to provide services in response to P1’s identified needs. In an incident report completed on July 4, 2022 for P1, the license holder stated that the staff person (SP1) working with P1 on July 4, 2022 did not provide supervision to P1 at all times while P1 was outside of their home. The incident report stated that SP1 had no alone time and staff should have followed P1 outside. SP1 stayed in P1’s apartment while P1 was outside. P1 and a neighbor got into a verbal altercation. P1 called 911 and was transported to the hospital for psychiatric care.
SP1 has not worked with P1 since this date.
Repeat Violation: The license holder was cited for a similar violation in an Order of License Revocation, Determinations of Maltreatment, and Failure to Report Maltreatment dated June 27, 2022 and correction orders dated December 29, 2017, June 14, 2019, August 19, 2020, and May 20, 2021.
Corrective Action Ordered: Because the license is currently revoked, operating under appeal, within 30 days, you must: · share this correction order with the person, and the person’s case manager;
· and inform each of how you will maintain compliance with this licensing requirement on a continuing basis; and
· submit, to the licensor identified in this correction order, evidence showing that the above mentioned requirements has been completed.
Additionally, you must ensure SP1 is provided training on P1’s identified needs, P1’s CSSPA and how to implement P1’s CSSPA prior to working with P1 in the future. On an ongoing basis, you must maintain compliance as required.
2. Citation: Minnesota Statutes 245D.081, subdivision 3.
Violation: The license holder did not ensure the designated manager provided program management and oversight of the services provided by the license holder as required.
The designated manager identified by the license holder failed to provide program management and oversight of the services provided by the license holder when the designated manager did not ensure SP1 was provided with training on the following prior to SP1 having unsupervised direct contact with P1: · P1’s coordinated service and support plan (CSSP) or coordinated service and support plan addendum (CSSPA) as it related to the responsibilities assigned to the license holder; and
· P1’s individual abuse prevention plan (IAPP), to achieve and demonstrate an understanding of the person as a unique individual, and how to implement those plans.
Corrective Action Ordered: Because the license is currently revoked, operating under appeal, within 30 days, you must: · share this correction order with the person, and the person’s case manager;
· and inform each of how you will maintain compliance with this licensing requirement on a continuing basis; and
· submit, to the licensor identified in this correction order, evidence showing that the above mentioned requirements has been completed. On an ongoing basis, you must provide program management and oversight as required in this subdivision.
3. Citation: Minnesota Statutes, section 245D.09, subdivision 4a.
Violation: For one staff person whose record was reviewed (SP1), the license holder did not provide orientation to individual service recipient needs as required.
The license holder failed to provide training to SP1 on the individual service recipient needs for P1 prior to SP1 having unsupervised direct contact with P1 on July 4, 2022. SP1 was unable to provide services in accordance with P1’s needs, because the license holder failed to provide training to SP1 on the following: · person’s coordinated service and support plan (CSSP) or coordinated service and support plan addendum (CSSPA) as it related to the responsibilities assigned to the license holder;
· the person’s individual abuse prevention plan (IAPP), to achieve and demonstrate an understanding of the person as a unique individual, and how to implement those plans.
SP1 has not worked with P1 since this date.

Corrective Action Ordered: Because the license is currently revoked, operating under appeal, within 30 days, you must: · share this correction order with the person, and the person’s case manager;
· and inform each of how you will maintain compliance with this licensing requirement on a continuing basis; and
· submit, to the licensor identified in this correction order, evidence showing that the above mentioned requirements has been completed.
Additionally, you must ensure SP1 is provided training on P1’s identified needs and P1’s CSSP, CSSPA, and IAPP prior to working with P1 in the future. On an ongoing basis, you must ensure all staff are provided orientation to individual service recipient needs before having unsupervised direct contact with a person served by your program as required.
If you fail to correct the violations specified in the Correction Order within the prescribed time lines the Commissioner may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07.

B. Right to Request Reconsideration
If you believe any of the citations are in error, you have the right to request that the Commissioner of Human Services reconsider the parts of the Correction Order that you believe to be in error. The request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this report. Your request for reconsideration must be sent to:

Please note that a request for reconsideration does not stay any provisions or requirements of the Correction Order. The Commissioner’s disposition of a request for reconsideration is final and not subject to appeal under Minnesota Statutes, chapter 14.
