CORRECTION ORDER
Dear Blake Elliott:
On October 12, 2022, a licensing complaint investigation of Bridges MN located at 1411 Shakopee Avenue East, Shakopee, Minnesota, was conducted to determine compliance with state and federal laws and rules governing the provision of home and community-based services to persons with disabilities and age 65 and older under Minnesota Statutes, Chapter 245D. As a result of this licensing complaint investigation a Correction Order is being issued.
A. Reason for Correction Order
Pursuant to Minnesota Statutes, section 245A.06, if the Commissioner of the Department of Human Services (DHS) finds that the license holder has failed to comply with an applicable law or rule and this failure does not imminently endanger the health, safety, or rights of the persons served by the program, the Commissioner may issue a Correction Order to the license holder.
The following violation(s) of state or federal laws and rules were determined as a result of the licensing review. Corrective action for each violation is required by Minnesota Statutes, section 245A.06 and is hereby ordered by the Commissioner of Human Services.
1. Citation: Minnesota Statutes, section 245D.07, subdivision 1a.
Violation: For three of three persons whose records were reviewed (P1, P2 and P3), the license holder did not provide services in response to the person’s identified needs as required.
a. On September 2, 2022, P1 and P2 were unsupervised at the CRS – 1108950 left alone in the home for approximately 20 minutes. P1 and P2 both have a 24 hour care plan without any alone time. SP1 left the home at around 1:40 in the afternoon and SP2 did not arrive until 2:00, leaving P1 and P2 in the home alone. SP1 claimed to have been in their car making a phone call; however, there was no other car at the facility and SP1 was not onsite.
b. On September 16, 2022, SP1 was scheduled to provide direct care services at CRS – 1108950. At approximately 2:30 PM, P3’s day service staff attempted to drop off P3 at home. The door to the home was locked, so the day service staff person rang the doorbell, knocked on the door, and called the land line phone several times and no one answered. The day service staff called their supervisor, who advised them to take P3 with them on the rest of the route and come back while the supervisor attempted to call the license holder’s (Bridges MN) emergency numbers. P3 and the day service staff person returned to the home at 3:00 PM and attempted to knock, ring the door bell, and call the landline in the home; however, no one answered. At approximately 3:30, P3’s guardian came to the CRS and picked P3 up.
Corrective Action Ordered: P1, P2 and P3 no longer receive services from the license holder. SP1 was terminated on September 28, 2022. On an ongoing basis, you must maintain compliance as required in this subdivision.
2. Citation: Minnesota Statutes, section 245D.081, subdivision 2.
Violation: The license holder did not provide oversight as required.
On September 16, 2022, SP3 stated a human recourses staff person called SP1’s cell phone between 3:00 and 3:15 to verify SP1 was at the home when the day service staff were attempting to drop off P3. SP1 stated they were at the facility; however, the designated coordinator or the designated manager did not call the landline in the CRS or went to the CRS to verify SP1 was present and providing direct care.
Corrective Action Ordered: On an ongoing basis, you must provide the program management and oversight as required in this subdivision.
B. Right to Request Reconsideration
If you believe any of the citations are in error, you have the right to request that the Commissioner of Human Services reconsider the parts of the Correction Order that you believe to be in error. The request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this report. Your request for reconsideration must be sent to:

Please note that a request for reconsideration does not stay any provisions or requirements of the Correction Order. The Commissioner’s disposition of a request for reconsideration is final and not subject to appeal under Minnesota Statutes, chapter 14.
C. Posting the Correction Order Pursuant to Minnesota Statutes, section 245A.06 subdivision 8, you must post this Correction Order for two years in a place that is conspicuous to the people receiving services and all visitors to the facility, even if you request reconsideration.
