That the maltreatment determination for neglect of Child AV, as set forth in this matter against Creative Stars Academy LLC is REVERSED; and that the Order to Pay a Fine issued to Creative Stars Academy LLC in the amount of $1000.00 for neglect of Child AV is RECINDED; 3. The determination that Creative Stars Academy LLC is responsible for Failure to Report Maltreatment is AFFIRMED; and the Order to Pay a Fine issued to Creative Stars Academy LLC in the amount of $200.00 for failure to report maltreatment is AFFIRMED; 4. The determination that Creative Stars Academy LLC is responsible for failing to request a background study is AFFIRMED; and the Order to Pay a Fine issued to Creative Stars Academy LLC in the amount of $200.00 for failure to request a background study is AFFIRMED; 5. The determination that Creative Stars Academy LLC is responsible for failing to ensure adequate supervision is AFFIRMED; 6. The determination that Creative Stars Academy LLC is responsible for failing to create an individual child care plan for a child with special needs is AFFIRMED; and 7.
