Dear Blake Elliott On November 8, 2022, a Department of Human Services (DHS) licensor conducted a licensing investigation at your facility located at 1932 University Ave W, St Paul. As a result of this visit, DHS determined that you are in violation of three of the 245D Home and Community Based rules and statutes. As a result, DHS is issuing this order, which requires you to take the correction action as described below. Details of our findings are provided below. Our next steps and your options are also detailed. LICENSING VIOLATIONS
DHS determined that your program failed to follow licensing rules and statutes, as described below. SERVICE RECIPIENT
1. Violation: For one of one person whose record was reviewed (P1), the license holder did not provide services in response to P1’s identified needs.
On August 23, 2022 from 3PM-9PM, Bridges MN failed to ensure staff was available for P1 as required in the “Coordinated Service and Support Plan Addendum-Intensive Services” dated October 18, 2021.
Statute Violated: section 245D.07, subdivision 1a
Repeat Violation: In Correction Orders that DHS issued on July 30, 2019; March 22, 2019; May 20, 2021; October 15, 2021; December 6, 2021; February 25, 2022; June 8, 2022; August 19, 2022; September 29, 2022; November 4, 2022; and November 10, 2022 you were previously found in violation of this same statute.
2. Violation: For P1, the license holder did not maintain service recipient records as required.
The license holder failed to maintain an updated medical appointment schedule as required. Bridges MN was responsible for assisting P1 to schedule, transport and attending all medical appointments. P1’s medical visit log dated 2/5/2020 failed to document all medical visits as required. Statute Violated: section 245D.095, subdivision 3. 3. Violation: The license holder did not implement medication administration procedures to ensure P1 took medications and treatment as prescribed.
Through interviews with SP1, SP2 and a review of P1’s support plan, and Bridges MN was assigned responsibility for maintaining contact with P1 prescriber, obtaining P1’s medication through the assigned pharmacy and administering medications and treatments to P1. Between June 1, 2022 and July 6, 2022, Bridges MN failed to fill P1’s prescriptions, which resulted in P1 unable to take their medications as prescribed. Statute Violated: section 245D.05, subdivision 2, paragraph (b). Repeat Violation: In Correction Orders that DHS issued on March 10, 2020; October 7, 2020; February 5, 2021; April 2, 2021 and November 9, 2021 you were previously found in violation of this same statute. Corrective action required
Within 30 days of this order, because the license is currently revoked, operating under appeal, you must share this correction order with the person, the person’s legal representative, and the person’s case manager and inform each of them how you will maintain compliance with this licensing requirement on a continuing basis. On an ongoing basis, you must maintain compliance as required with this subdivision.
