CORRECTION ORDER
Dear Blake Elliott:
A licensing complaint investigation of Bridges MN, located at 1932 University Avenue West, Saint Paul, Minnesota was conducted to determine compliance with state and federal laws and rules governing the provision of home and community-based services to persons with disabilities and age 65 and older under Minnesota Statutes, Chapter 245D. As a result of the licensing complaint investigation(s) a Correction Order is being issued.
A. Reason for Correction Order
Pursuant to Minnesota Statutes, section 245A.06, if the Commissioner of the Department of Human Services (DHS) finds that the license holder has failed to comply with an applicable law or rule and this failure does not imminently endanger the health, safety, or rights of the persons served by the program, the Commissioner may issue a Correction Order to the license holder.
The following violation(s) of state or federal laws and rules were determined as a result of the licensing review. Corrective action for each violation is required by Minnesota Statutes, section 245A.06 and is hereby ordered by the Commissioner of Human Services.
1. Citation: Minnesota Statutes, section 245D.05, subdivision 1, paragraph (a).
Violation: For one of one person whose record was reviewed (P1), the license holder failed to meet the person’s health needs as assigned in the support plan or support plan addendum.
P1’s support plan addendum, dated May 4, 2022, stated that the license holder was responsible for P1’s health needs and that staff “will schedule, transport and attend all of [P1’s] medical and dental appointments. Staff will ensure that any prescribed treatment plan is correctly implemented.” The license holder failed to meet P1’s health needs when:
· the license holder did not ensure P1 attended routine lab appointments that were required for the refill of one of P1’s medications, Vancomycin Solution, and it resulted in P1’s medication not being refilled. P1 was without the daily prescribed medication for two months. The license holder was unable to determine if staff attempted to schedule the lab appointment or communicated with the prescriber; and
· one of P1’s prescriptions, Omeprazole Suspension, was switched to a different pharmacy to be filled. The pharmacy had the incorrect contact information for the license holder. P1 was without this daily scheduled medication for two months. The license holder was unable to determine if staff attempted to follow up with the prescriber. The license holder stated that staff said that they called the pharmacy regarding this issue, but there was no documentation of these attempts to contact the pharmacy.
Corrective Action Ordered: P1 no longer receives services from the license holder. On an ongoing basis, you must maintain compliance as required in this subdivision.
2. Citation: Minnesota Statutes, section 245D.05, subdivision 2, paragraph (b).
Violation: For one person whose record was reviewed (P1), the license holder did not implement medication procedures as required.
a. The license holder failed to ensure P1 took medications as prescribed. The license holder failed to administer two of P1’s medications, Omeprazole Suspension and Vancomycin Solution, from August 12, 2022 through October 12, 2022.
In addition, P1’s Divalproex dosage was increased on September 8, 2022 and also increased again on October 7, 2022. The license holder failed to start administering the increased dosages and administered incorrect doses of the medication from September 8, 2022 through October 11, 2022.
b. The license holder provided intensive support services. The license holder failed to administer medications to P1 according to the license holder’s medication administration policy and procedures as required under section 245D.11, subdivision 2, clause (3).
Repeat Violation: The license holder was cited for a similar violation in an Order of License Revocation, Determinations of Maltreatment, and Failure to Report Maltreatment dated June 27, 2022; and in correction orders dated march 10, 2020, October 7, 2020, February 5, 2021; April 2, 2021; November 9, 2021; November 22, 2022; and November 30, 2022
Corrective Action Ordered: P1 no longer receives services from the license holder. On an ongoing basis, you maintain compliance as required in this subdivision.
3. Citation: Minnesota Statutes 245D.081, subdivision 2.
Violation: The license holder did not ensure the designated coordinators provided coordination of the service delivery and evaluation of the services provided by the license holder as required.
The designated coordinator identified by the license holder failed to provide oversight to ensure P1’s services were delivered as required, including meeting P1’s health needs and administering P1’s medications.
Repeat Violation: The license holder was cited for a similar violation in an Order of License Revocation, Determinations of Maltreatment, and Failure to Report Maltreatment dated June 27, 2022; and in correction orders dated November 10, 2022, November 4, 2022, October 14, 2022, June 3, 2022, May 20, 2021, and June 14, 2019.
Corrective Action Ordered: On an ongoing basis, you maintain compliance as required.
If you fail to correct the violations specified in the Correction Order within the prescribed time lines the Commissioner may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07.
B. Right to Request Reconsideration
If you believe any of the citations are in error, you have the right to request that the Commissioner of Human Services reconsider the parts of the Correction Order that you believe to be in error. The request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this report. Your request for reconsideration must be sent to:

Please note that a request for reconsideration does not stay any provisions or requirements of the Correction Order. The Commissioner’s disposition of a request for reconsideration is final and not subject to appeal under Minnesota Statutes, chapter 14.
