Dear Blake Elliott:
CORRECTION ORDER
An investigation of Bridges Birch Lake was completed regarding report number 202209833 which alleged violation of Minnesota Statutes, section 626.557, and Minnesota Statutes, Chapter 245D. This maltreatment allegation concerned financial exploitation. The disposition of the investigation report is substantiated with inconclusive responsibility. One violation was noted.
A. Reason for Correction Order
The following violation of state and (or) federal laws and rules was observed. Corrective action for the violation is required by Minnesota statutes section 626.557, subdivision 3, paragraph (a), and is hereby ordered by the Commissioner of Human Services. Failure to correct the violation within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07.
Citation: Minnesota statutes section 626.557, subdivision 3, paragraph (a), states that “a mandated reporter who has reason to believe that a vulnerable adult is being or has been maltreated . . . . . . shall immediately report the information to the common entry point.”
Violation: A mandated reporter shall immediately make a report to the common entry point.
During the course of the investigation, it was determined that on November 18, 2022, staff persons at multiple levels of authority were aware that a client’s debit card was missing and that an unauthorized withdrawal was made. The Department of Human Services did not receive a report until November 30, 2022.
Repeat Violation: The license holder was cited for a similar violation in an Order of License Revocation, Determinations of Maltreatment, and Failure to Report Maltreatment dated June 27, 2022; and in an Order to Pay a Fine dated December 27, 2019.
Correct Action Ordered: Within 30 days from receipt of this order, submit a plan to your licensor explaining how you will maintain compliance in the future. Because the license is currently revoked, operating under appeal, you must share this correction order with the person, the person’s legal representative, and the person’s case manager and inform each of how you will maintain compliance with this licensing requirement on a continuing basis.
B. Right to Reconsideration
If you believe any of the citations are in error, you may ask the Commissioner of Human Services to reconsider the parts of the correction order that you believe to be in error. If you choose to exercise this right, your request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this letter. Your request for reconsideration must be sent to:

You must include any and all information, documentation, or other evidence you have to support your request for reconsideration. Please note that a request for reconsideration does not stay any provisions of the correction order.
