CORRECTION ORDER

An investigation of Integrity Living Options-Yukon Site was completed regarding report number 202300018 which alleged violation of Minnesota Statutes, section 626.557, and Minnesota Statutes, Chapter 245D. This maltreatment allegation concerned neglect. The disposition of the investigation report is inconclusive. One violation was noted.

VIOLATIONS AND CORRECTION ORDERS

The following violation of state and (or) federal laws and rules was observed. Corrective action for the violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violation within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07.

  Citation: Minnesota Statutes section 245D.07, subdivision 1.

Violation: The license holder did not provide services as assigned in the support plan.

A client’s (C’s) support plan and support plan addendum stated, “[The C] has a history of noncompliance … will refuse showers and isolate in [her/her] room. Staff will provide verbal reminders to [the C] as needed … Staff additionally will document all [the C’s] refusals … and report these concerns to [the C’s] team should they arise.”

A staff person (S) provided information that the C typically “refused” cares and/or to follow the doctor’s or nurse’s recommendations. The facility’s progress notes did not contain documentation of all the C’s “refusals” and there was no documentation of concerns related to this being reported to the C’s team.

Corrective Action Ordered: Correct immediately and maintain compliance with this licensing requirement on a continuing basis throughout the program.

If you believe any of the citations are in error, you may ask the Commissioner of Human Services to reconsider the parts of the correction order that you believe to be in error. If you choose to exercise this right, your request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this letter.
