CORRECTION ORDER
Dear Marlin: On March 9, 2023, a Department of Human Services (DHS) licensor received information regarding your facility located at 6043 Hudson Road Suite 220, Woodbury, MN 55125. As a result of this information, DHS determined that you are in violation of the Substance Use Disorder Treatment rules and statutes. As a result, DHS is issuing this order which requires you to take the correction action as described below. Details of our findings are provided below. Our next steps and your options are also detailed. LICENSING VIOLATIONS
DHS determined that your program failed to follow licensing rules and statutes, as described below. 1. Violation: The program failed to follow the terms of a variance which was granted on March 14, 2022 to allow clients to be admitted to the license holder's opiate treatment program and begin receiving services without a government issued identification. The program did not document monthly attempts made to assist clients in obtaining a government issued photo ID card.
Statute Violated: Minnesota Statutes, sections 245A.04, subdivision 9, paragraph (a), and 245G.22, subdivision 14, paragraph (a).
Corrective action required
On March 16, 2023, you were granted a revised variance to allow clients to be admitted to the opiate treatment program and begin receiving services without a government issued identification. You must follow the terms identified in that variance. YOUR RIGHT TO REQUEST RECONSIDERATION
