PUBLIC Summary of Settlement Agreement License Holder: Dulmar Hussein Mohamed License Number: 1062959 Program Type: Family Child Care Date of Licensing Action: October 5, 2022 Licensing Action Taken: Order of Revocation Action appealed and settlement agreement signed by Commissioner on: May 2, 2023 Terms of Settlement Agreement: License will be placed on conditional status for two years, with the following terms: a. Follow and comply with all applicable Minnesota Rules and Laws. b. No variances to age distribution or capacity will be granted during the conditional period. c. License Holder’s child care license will be converted to a Class A license (subject to allow for care up to ten (10) children total, with only six (6) of the children being under the age of five). d. License Holder shall only operate her daycare for 16 hours maximum per a 24-hour period. License Holder ’s 16 hours of operation shall consist of only daytime hours with no overnight care. e. License Holder shall obtain services of a mentor through the program that has already been approved by Olmsted County prior to the date of this Agreement or other similar programs to be approved by Olmsted County, focusing on behavior guidance techniques. f. License Holder will complete six (6) hours of additional training on topics recommended by the Agency. g. License Holder shall complete the licensing recertification process and provide documentation that she is current on her annual training prior to reopening her daycare.
