August 18, 2023 Michelle Murray 505 Highway 169 North, Suite 500 Plymouth, MN 55441 RE: Report No. 202304093 Nexus Gerard Family Healing License No. 831080 Dear Michelle Murray: CORRECTION ORDER An investigation of Nexus Gerard Family Healing was completed regarding report number 202304093 which alleged violation of Minnesota Statutes, section 260E, and Minnesota Rules, parts 2960.0010 to 2960.0120 (CRF). This maltreatment allegation concerned neglect. The disposition of the investigation report is maltreatment not determined. One violation was noted. VIOLATIONS AND CORRECTION ORDERS The following violation of state and (or) federal laws and rules was observed. Corrective action for the violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violations within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07. Citation: Minnesota Rules, chapter 2960.0080, subdivision 17, item A. Violation: The license holder must report critical incidents of a serious nature that involve or endanger the life or safety of the resident or others to the commissioner of human services or corrections within ten days of the occurrence. On May 11, 2023, eight youth were walking through a nature center and came to a flooding creek. Four youth went into the water and were caught in the current. The four youth and a staff person went through a culvert to get out of the current. The facility did not report an incident as required. Corrective Action Ordered: Within 30 days from receipt of this order, submit a plan to your licensor explaining how you will maintain compliance in the future.
