CORRECTION ORDER
Dear Blake Elliott: On August 14, 2023, a Department of Human Services (DHS) licensor reviewed a serious injury report submitted by Bridges MN. As a result of this report, DHS determined that you are in violation of one of the Home and Community Based Services—245D rules and statutes. As a result, DHS is issuing this order which requires you to take the correction action as described below. Details of our findings are provided below. Our next steps and your options are also detailed. LICENSING VIOLATIONS
DHS determined that your program failed to follow licensing rules and statutes, as described below. Incident response and reporting
1. Violation: For one of one person whose record was reviewed (P1), the license holder did not report a serious injury as required.
P1 substained a serious injury on July 29, 2023. The license holder became aware of the serious injury on August 11, 2023. The license holder failed to report the injury within 24 hours. The license holder reported the serious injury on August 14, 2023. Statute Violated: Minnesota Statutes, section 245D.06, subdivision 1, paragraph (e).
Repeat Violation: In a Correction Order that DHS issued on November 9, 2021, December 31, 2020; August 14, 2020 you were previously found in violation of this same statute. Corrective action required
Because the license is currently revoked, operating under appeal, you must:
· share this correction order with P1, P1’s legal representative (if applicable), and case manager;
· inform each of the above mentioned people, how you will maintain compliance with this licensing requirement on a continuing basis;
· submit to the licensor evidence showing that the above-mentioned requirements have been completed; and
· maintain evidence in P1’s record.
On an ongoing basis, you must maintain compliance as required.

· List each violation you are challenging and identify what is inaccurate or incomplete about the information in the order
· Supply information that is accurate or more complete

If you do not meet this deadline, you lose your right to request reconsideration. The timeline to appeal began when you received this order.
