PUBLIC Summary of Settlement Agreement License Holder: Bonnie Miley and Jody Jaakola License Number: 1021243 Program Type: FCC Date of Licensing Action: October 14, 2022 Licensing Action Taken: Order of Revocation Action appealed and settlement agreement signed by Commissioner on: September 5, 2023 Terms of Settlement Agreement: License will be placed on conditional status for two years with the following terms: a. Follow and comply with all applicable Minnesota Rules and Laws. b. No variances to age distribution or capacity will be granted during the conditional period. c. License Holders’ child care license will be converted to a Class A license (subject to allow for care up to ten (10) children total, with only six (6) of the children being under the age of five). d. Within 90 days of execution of this settlement agreement, License Holders will provide proof to Dakota County that they completed: (i) at least six hours of additional training related to sanitation and health requirements and (ii) at least four hours of training related to caring for children with special needs. Dakota County must pre-approve these training courses. These training requirements are in addition to the annual training requirements as listed in Minnesota Statutes, section 245A.50. e. License Holders shall obtain the services of a mentor through the program that has been approved by Dakota County, focusing on supervision; sanitation and health; and caring for children with special needs. License Holders agree to work cooperatively with their mentor for the duration of the mentorship program and follow the recommendations of the mentor. License Holders are responsible for any fees that may arise during the course of the mentorship program. License Holders agree to report their progress with the mentor to the county licensor, including sharing a copy of any plans for compliance. f. Within 30 days of execution of this settlement agreement, License Holders will pay a fine in the amount of $200 for a violation of Minnesota Statutes, Chapter 245A.07, subdivision 3(a)(3).
