Terms of Settlement Agreement:

License Holder’s family child care license shall be placed on conditional status for a period of two years. While under the conditional license, License Holder shall comply with the following terms:

a.  Follow and comply with all applicable Minnesota Rules and Laws.

b.  No variances to age distribution or capacity will be granted during the conditional period.

c.  License Holder shall obtain services of a mentor through a program that has been approved by Morrison County, focusing on safety and supervision. License Holder agrees to work cooperatively with her mentor for the duration of the mentorship program and follow the recommendations of the mentor. License Holder is responsible for any fees that may arise during the course of the mentorship program. License Holder agrees to share her progress with the mentor to her family child care licensor.

d.  License Holder shall immediately sign up for the routine email updates provided by Sourcewell licensors and implement any changes that are required as a result of the information received.

e.  Prior to resuming child care, License Holder shall collect and maintain immunization records for all children in her care and continue to do so when new children enroll in her family child care program.

f.  License Holder shall complete a six-hour supervising for safety training course within 30 days of execution of this settlement agreement. This training is in addition to the annual training requirements listed in Minnesota Statutes, section 245A.50. Prior to attending the training, License Holder must obtain approval from Morrison County that the training is appropriate. License Holder must submit documentation of her course attendance to Morrison County.

g.  License Holder must provide a copy of this Agreement to parents of children in care or document that all parents have been given an opportunity to review the Agreement. License Holder must obtain parent signatures for each currently enrolled child, verifying they have either received a copy of the Agreement or had an opportunity to review the Agreement. License Holder must provide this documentation to Morrison County within 30 days of execution of this agreement. For new families, License Holder must submit documentation of compliance with this term to Morrison County within 5 days of any child’s admission to her child care program.
