PUBLIC Summary of Settlement Agreement License Holder: Jessica Jacobson License Number: 1092888 Program Type: Family Child Care Date of Licensing Action: May 17, 2023 Licensing Action Taken: Order of Revocation Action appealed and settlement agreement signed by Commissioner on: October 11, 2023 Terms of Settlement Agreement: 1. The License Holder’s family child care license shall be placed on conditional status for a period of two years from the effective date of this Settlement Agreement. While under the conditional license, the License Holder shall comply with the following terms: a. Follow and comply with all applicable Minnesota Rules and Laws. b. No variances to age distribution or capacity will be granted during the conditional period. c. The License Holder must ensure that the household member identified in the Non-Public Provider Agreement does not have direct contact with persons served by the family child care program, except for their own children who are enrolled in the family child care program. d. The License Holder must ensure that the household member identified in the Non-Public Provider Agreement does not act as a caregiver or a substitute caregiver in the family child care program and that they do not transport persons served by the family child care program, except for their own children who are enrolled in the family child care program. e. The License Holder will immediately report any changes in her household and in the household member’s residence and employment status to the County Licensor. f. If License Holder learns that the household member violates any of the terms of their variance, the License Holder must immediately notify Clay County Family Childcare License staff of the violation. g. The License Holder must comply with all applicable Minnesota Statutes and Rules, including Minnesota Rules, parts 9502.0300 through 9502.0445.
