Terms of Settlement Agreement:

1. The License Holder’s family child care license shall be placed on conditional status for a period of one year from the effective date of this Settlement Agreement.  While under the conditional license, the License Holder shall comply with the following terms:

a. Within 30 calendar days of execution of this settlement, License Holder must complete a six-hour Supervising for Safety training course, either in-person or online, and provide documentation to Kandiyohi County. This provision is in addition to the annual training requirements as listed in Minnesota Statutes, section 245A.50.

b. Within 30 days of execution, License Holder will provide Kandiyohi County with a detailed written plan that addresses how she will provide outdoor supervision to children in care, including during transition times. This plan must be approved by Kandiyohi County. License Holder agrees to comply with the approved supervision plan at all times.

c. License Holder must comply with all applicable Minnesota Statutes and Rules, including Minnesota Statutes, chapters 245A and 245C and Minnesota Rules, parts 9502.0300 through 9502.0445.

d. No other variances as to age or capacity will be granted during the period of conditional license.
