PUBLIC Summary of Settlement Agreement License Holder: Kathleen Cornell License Number: 1051382 Program Type: Family Child Care Date of Licensing Action: June 9, 2023 Licensing Action Taken: Order of License Revocation Action appealed and settlement agreement signed by Commissioner on: November 14, 2023 Terms of Settlement Agreement: 1. DHS agrees to remove the individual identified in the Non-Public Provider Agreement from License Holder’s family child care household roster, license number 1051382. 2. Upon final execution of this Settlement Agreement the License Holder shall comply with the following terms for as long as the individual identified in the Non-Public Provider agreement is disqualified under Minnesota Statutes, Chapter 245C: a. Follow and comply with all applicable Minnesota Rules and Laws. b. The License Holder must ensure that the individual identified in the Non-Public Provider Agreement does not have any direct contact with, or access to, persons served by the family child care program. c. The License Holder must ensure that the individual identified in the Non-Public Provider Agreement is not present in the family child care home and has no contact with children served at least 30 minutes before the program opens and no less than 30 minutes after the program closes, or any other time when License Holder provides family child care services. d. The License Holder will immediately report any known address changes for the individual identified in the Non-Public Provider Agreement to her County Licensor.
