PUBLIC Summary of Settlement Agreement License Holder: Elsa Wodesso Parent License Number: 1071238 HCBS and 1098292 AFC Program Type: Home and Community-Based Services (HCBS) and Adult Foster Care Date of Licensing Action: April 5, 2023 Licensing Action Taken: License Revocation Action appealed and settlement agreement signed by Commissioner on: November 20, 2023 Terms of Settlement Agreement: License holder withdrew its appeal of the Order of Revocation. DHS agreed not to impose the order revoking the licenses. License Holder can continue to operate for 60 calendar days from the final execution of the Settlement Agreement. The License Holder agreed to surrender the licenses upon the transition of all persons served to new service providers or within 60 calendar days, whichever occurs first. The License holder and all controlling individuals agreed not to reapply or hold a license to provide any type of services licensed by DHS for at least four years from the final execution of the Agreement.
