December 20, 2023 Sharon Evans 4629 Oakland Avenue Minneapolis, MN 55407-3535 License number: 226305 FCC ORDER LIFTING IMMEDIATE SUSPENSION Dear Ms. Evans: In an order dated November 15, 2023, the Commissioner of the Department of Human Services temporarily immediately suspended your license to provide family child care due to a determination that the health, safety, and rights of children in your care were in imminent risk of harm. Based upon information received, the Department of Human Services is lifting the previous Order of Temporary Immediate Suspension. Therefore, effective immediately, you may resume operation of your family child care program located at 4629 Oakland Avenue, Minneapolis, MN, in accordance with the terms of your license.
