PUBLIC Summary of Settlement Agreement License Holder: Aldrich Boarding Care Home, LLC, DBA Bridges MN License Number: 1079030 Program Type: 245D-HCBS Date of Licensing Action: June 27, 2022 Licensing Action Taken: Order of License Revocation, Determinations of Maltreatment, and Failure to Report Maltreatment Action appealed and settlement agreement signed by Commissioner on: January 29, 2024 Terms of Settlement Agreement: · The License Holder agreed to withdraw its appeal. · DHS agreed not to impose or enforce the Order of License Revocation. · The parties agreed the maltreatment determinations are conclusive and that no fines or further sanctions would be imposed. · DHS agreed not to impose a fine for a Dakota County maltreatment determination against Bridges MN Carrington. · The majority of License Holder’s CRS satellite licenses were closed effective August 31, 2023 following an acquisition by another provider, and License Holder voluntarily surrendered the Parent License and all satellite licenses no longer operated by Bridges MN as of September 1, 2023. · The License Holder agreed not to submit an application to DHS to provide any licensed services or supports for a period of five years from September 1, 2023. · Controlling Individuals, Blake Elliot, Bach Parker, Scott Loe, Paul Gans, and Brian Zirbes agreed not to apply for or hold a license or serve as a controlling individual for any DHS licensed program for a period of five years from September 1, 2023. The settlement does not prohibit these individuals from being employed or otherwise engaged by any other DHS0licensed program so long as they do not engage in activities that meet the definition of a controlling individual.
