PUBLIC Summary of Settlement Agreement License Holder: Jessica Jacobson License Number: 1092888 Program Type: Family Child Care Date of Licensing Action: May 6, 2024 Licensing Action Taken: Order of Temporary Immediate Suspension Action appealed and settlement agreement signed by Commissioner on: July 30, 2024 Terms of Settlement Agreement: The License Holder’s family child care license shall be placed on conditional status for a period of two years. While under the conditional license, the License Holder shall comply with the following terms and additional terms listed in the Non-Public Provider Agreement: a. The individual identified in the non-public provider agreement will be removed from the license as a household member. b. The License Holder must ensure that the individual identified in the non-public provider agreement will not be allowed on the licensed child care property. c. The License Holder must ensure that the individual identified in the Non-Public Provider Agreement does not have any direct contact with, or access to, persons served by the family child care program. d. The License Holder must comply with all applicable Minnesota Statutes and Rules, including Minnesota Rules, parts 9502.0300 through 9502.0445.
