August 30, 2024 Kathryn Harris, Authorized Agent Alliance Wellness Clinic Inc. 8040 Old Cedar Ave S #100 Minneapolis, MN 55425 License Number: 1091777 Report Number: 202405074 CORRECTION ORDER Dear Kathryn Harris: A Department of Human Services (DHS) licensor conducted a licensing investigation of your facility, Alliance Wellness Clinic Inc located at 8040 Old Cedar Ave S Ste #100, Minneapolis, MN, 55425. This investigation was conducted to determine compliance with state and federal laws and rules governing the provision of substance use disorder treatment services under Minnesota Statutes 245G. As a result, DHS is issuing this order which requires you to take the correction action as described under each violation. Details of our findings are provided below. Our next steps and your options are also detailed. LICENSING VIOLATIONS DHS determined that your program did not follow licensing rules and statutes, as described below. Policy and Practice 1. Violation: The license holder’s policy and practice did not ensure compliance with the requirements of Code of Federal Regulations, title 42, part 8. On May 25, 2024 the license holder did not have a practitioner licensed under the appropriate State law and registered under the appropriate State and Federal laws to administer or dispense medication for opioid use disorder. The program reported the following number of clients were not able to be administered their medication as prescribed and/or provided their take home dose(s): a. Eighteen of forty-nine clients Saturday, May 25, 2024; b. Nineteen of forty-nine clients Sunday, May 26, 2024; and c. Twenty-four of forty-nine clients Monday, May 27, 2024. Rule Violated: Minnesota Statues, section 245G.22, subdivisions 1, paragraph (a); and 17, paragraph (b). Corrective Action Required: Immediately and on an ongoing basis, the license holder’s policy and practice must ensure compliance with the requirements of Code of Federal Regulations, title 42, part 8. Within 30 days of receipt of this order, submit a clinic closure policy that demonstrates policy. Written Response Required If you fail to correct the violation(s) specified in the Correction Order within the prescribed time lines the Commissioner may issue an Order of Conditional License or may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07.
