CORRECTION ORDER

An investigation of REM River Bluffs Inc - North Point was completed regarding report number 202407022 which alleged violation of Minnesota Statutes, section 626.557, and Minnesota Statutes, Chapter 245D. This maltreatment allegation concerned emotional abuse. The disposition of the investigation report is inconclusive. Two violations were noted.

VIOLATIONS AND CORRECTION ORDERS

The following violations of state and (or) federal laws and rules were observed. Corrective action for each violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violations within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07.

1. Citation: Minnesota Statutes, section 245D.095, subdivision 5, paragraph (a), clause (2).

Violation: The license holder failed to maintain a personnel record of one staff person (S1) that documented orientation and training on two persons’ served (P1’s and P2’s) support plans as required.

Corrective Action Ordered: Immediately provide documentation that S1 was trained on C1’s and C2’s support plans.

2. Citation: Minnesota Statutes, section 245D.04, subdivision 3, paragraph (a), clause (6).

Violation: The license holder failed to ensure that C2’s protection related rights, including being treated with courtesy and respect, were followed.

Information showed that a supervisory staff person (S2) commented on C2’s masturbation and called C2 a “dumbass.”

Corrective Action Ordered: Immediately and an ongoing basis maintain compliance with protection related rights.
