PUBLIC Summary of Settlement Agreement License Holder: Shannon Rooney License Number: 1122529 (in application) Program Type: Family Child Care Date of Licensing Action: July 11, 2024 Licensing Action Taken: Order of License Denial Action appealed and settlement agreement signed by Commissioner on: November 27, 2024 Terms of Settlement Agreement: 1. Upon final execution of the Settlement Agreement, the Parties agree that License Holder may continue her application for a family child care license at her designated home address. DHS will remove the disqualified household member identified in the Non-Public Provider Agreement from the household information upon verification that the disqualified individual has moved out of the designated address of the program. 2. If licensed for a family child care program, License Holder will follow the terms of the Non-Public Provider agreement and immediately notify Ramsey County of any changes in the household membership. 3. Nothing in this Agreement shall limit DHS’ authority to deny any pending or future license application if all licensing requirements are not met. Should DHS deny the application, License Holder maintains the right to appeal that order pursuant to Minnesota Statutes, section 245A.05, subdivision (b).
