The following violation of state and (or) federal laws and rules was observed. Citation:  Minnesota Statutes, section 245D.05, subdivision 2, paragraphs (b) and (c).

Violation: The license holder was responsible for medication administration of a client (C) and failed to ensure the C’s medications were given as prescribed and failed to ensure the C’s medication administration was documented accurately.

Information showed on January 9, 2024, the C’s Saphris order was changed by the C’s provider from as needed (PRN) to twice daily at 7 a.m. and 3 p.m. If the C did not need a dose of Saphris as outlined in the C’s Saphris Protocol, staff persons were to not administer Saphris and document “hold” in the C’s medication administration record. From January to May 2024, staff persons did not consistently document when Saphris was not administered to the C as “hold” and Saphris was not given twice daily to the C as specified on the order.

Corrective Action Ordered: Immediately and on an ongoing basis, ensure compliance with the licensing requirements within this statute.
