License Holder agrees to withdraw its request for reconsideration of the Correction Order dated June 2024. 2. DHS agrees to rescind the Determination of Maltreatment and Order to Pay a Fine, and issue an amended maltreatment investigation memorandum for Report No. 202402504 reflecting that rescission. 3. License Holder agrees that within 30 days of final execution of this Agreement, the program will submit written documentation to its DHS licensor detailing how the program will maintain compliance with Minnesota Rules, part 9503.0060, subparts 1, 4(B)(9), 5(B)(9), and 6(B)(7).
