CORRECTION ORDER
Dear Stephen Williams, On February 6th, 2025, the Department of Human Services (DHS) conducted a licensing investigation regarding your facility, Kadiri House, located at 2907 Newton Ave. N. Minneapolis, MN 55411. This investigation was conducted to determine compliance with state and federal laws and rules governing the provision of children’s residential facilities under Minnesota Rules, part 2960.0010 through 2960.0120 and children's group residential facilities under Minnesota Rules, parts 2960.0130 through 2960.0220. As a result of this investigation, DHS is issuing this order which requires you to take the correction action as described under each violation. Details of our findings are provided below. Our next steps and your options are also detailed. LICENSING VIOLATIONS
DHS determined that your program failed to follow licensing rules and statutes, as described below. Violation: The license holder utilized a restrictive procedure with a resident and the license holder was not certified to use restrictive procedures.
An incident report dated November 20, 2024, documented that a staff person (SP1) utilized a restrictive procedure on a resident (R1) by guiding R1 “to the ground.” Documentation suggested the presence of imminent risk, however, the facility was not certified to utilize restrictive procedures, the license holder did not have a restrictive procedures plan approved by the commissioner, and staff persons were not trained on the use of physical holding.
Rule Violated: Minnesota Rules, part 2960.0080, subpart 5, item E.
Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that the provisions governing restrictive procedures are not implemented at the program unless certification for the use is pursued. YOUR RIGHT TO REQUEST RECONSIDERATION

· This action is taken under Minnesota Statutes, section 245A.06, subdivision 1.
· This Children’s Residential Facility must maintain compliance with the licensing statutes and rules, specifically Minnesota Rules, chapter 2960.
