CORRECTION ORDER

Dear Ms. Lennick:

On February 24, 2025, a Department of Human Services (DHS) investigator conducted a licensing investigation regarding your facility located at 1089 10th Ave SE, Minneapolis, MN. As a result of this investigation, DHS determined that you are in violation of one of the child placing agency statutes. As a result, DHS is issuing this order which requires you to take the correction action as described below. Details of our findings are provided below. Our next steps and your options are also detailed.

LICENSING VIOLATIONS

DHS determined that your program failed to follow licensing rules and statutes, as described below.

1. Violation: In November 2024, your agency was directed by DHS to complete a licensing investigation on a licensing holder (LH1) regarding a report of several licensing violations. On February 18, 2025, it was reported to DHS that LH1 had applied at another agency for child foster care. When your agency was contacted by the other agency, your agency stated no licensing investigations had been completed regarding LH1. When you were contacted about this concern by DHS, you stated that your agency had failed to complete the licensing investigation as directed by DHS.

Statute Violated: Minnesota Statutes, section 245A.16, subdivision 2.

Corrective action required

You must correct the violations cited above. Submit documentation to your licensor within 30 days from when you received this order explaining how you have corrected the violations.

YOUR RIGHT TO REQUEST RECONSIDERATION

You have the right to request reconsideration of this order in writing. Your request must:

1. Specify the parts of the correction order that are alleged to be in error;

2. Explain why they are in error; and

3. Include documentation to support the allegation of error.

If you are mailing your request, it must be received by DHS within 20 calendar days from when you received this order. If you do not meet this deadline, you lose your right to request reconsideration. The timeline to appeal began when you received this order. Please send it to:

Legal authority for this licensing action

· This action is taken under Minnesota Statutes, section 245A.06, subdivision 1.

· This child placing agency/licensing agency must maintain compliance with the licensing statutes and rules, specifically Minnesota Rules, parts 9543.0010 to 9543.0150, Minnesota Rules, part 9545.0755 through 9545.0845, and Minnesota Statutes, Chapter 245A.

· The timeline to request reconsideration of the order is provided in Minnesota Statutes, section 245A.06, subdivision 2.
