CORRECTION ORDER
Dear Lewis Zeidner: A Department of Human Services (DHS) licensor conducted a licensing investigation of your facility, Twin Town Treatment Center dba EOSIS Twin Town located at 1706 University Ave W, Saint Paul, MN 55104. This investigation was conducted to determine compliance with state and federal laws and rules governing the provision of substance use disorder treatment services under Minnesota Statutes 245G. As a result, DHS is issuing this order which requires you to take the correction action as described under each violation. Details of our findings are provided below. Our next steps and your options are also detailed. LICENSING VIOLATIONS
DHS determined that your program did not follow licensing rules and statutes, as described below. Practice
1. Violation: The license holder’s practice for grievance procedure did not meet requirements. The license holder did not have documentation grievance dated October 31, 2023 was responded to within three days of receipt of the grievance.
Statute Violated: Minnesota Statutes, section 245G.15, subdivision 2.
Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure grievance procedure meets all applicable requirements.
Personnel File
2. Violation: One personnel file was reviewed for requirements governing orientation training (SP1) and did not meet requirements. The staff person did not complete orientation on the following:
a. Within 24 hours of starting work on the following topics:
1. Specific job duties;
2. Policies and procedures;
3. Client confidentiality; and
4. Client needs.
b. Within 72 hours of employment on the following:
1. Orientation to HIV minimum standards;
2. Maltreatment of vulnerable adults; and
3. The program abuse prevention plan.
Statute Violated: Minnesota Statutes, sections 245A.65, subdivision 3; 245G.13, subdivision 1.
Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure orientation training meets all applicable requirements. Client File
3. Violation: One client file was reviewed for requirements governing individual treatment plans (C1) and did not meet requirements. The individual treatment plan was not developed within ten days of service initiation.
Statute Violated: Minnesota Statutes, section 245G.06, subdivision 1.
Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure individual treatment plan meets all applicable requirements.
4. Violation: One client file was reviewed for requirements governing client record documentation requirements (C1) and did not meet requirements. The file did not contain documentation of a psychiatry appointment the client attended on October 30, 2023.
Statute Violated: Minnesota Statutes, section 245G.06, subdivision 2b, paragraph (b).
Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure client record documentation requirements meets all applicable requirements.
Written Response Required
If you fail to correct the violation(s) specified in the Correction Order within the prescribed time lines the Commissioner may issue an Order of Conditional License or may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07.
