PUBLIC Summary of Settlement Agreement License Holder: Trinh Duong License Number: 1114861 Program Type: Family Child Care Date of Licensing Action: December 5, 2024 Licensing Action Taken: Order of Conditional License and Order to Pay Fine Action appealed and settlement agreement signed by Commissioner on: April 29, 2025 Terms of Settlement Agreement: 1. License Holder voluntarily withdraws the appeal of the Order of Conditional License and Order to Pay a Fine. License Holder agrees that the Order of Conditional License dated December 5, 2024, and its terms will remain in effect upon her family child care license as specified in that Order. 2. License Holder agrees to pay $100.00 of the ordered fine to DHS and shall submit payment to DHS along with the executed Agreement. 3. DHS agrees to stay the remaining $100.00 of the ordered fine for a period of two (2) years from the date of final execution of the Agreement. The stayed fine shall be immediately reinstated if DHS determines that License Holder has failed to comply with licensing requirements or any existing conditions on license, or issues any additional orders to License Holder resulting in a fine, conditional license, or other licensing sanction during the period of the stay.
