An investigation of Meridian Services - Diamond Lake was completed regarding report number 202502567 which alleged violation of Minnesota Statutes, section 626.557, and Minnesota Statutes, Chapter 245D. VIOLATION AND CORRECTION ORDER

The following violation of state and (or) federal laws and rules was observed. Corrective action for the violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violation within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07.

Citation: Minnesota Statues, section 245A.04, Subdivision 13, paragraph (b), and paragraph (d), clauses (4) and (5).

Violation: The license holder failed to ensure separation of client funds from funds of the license holder, the program, or program staff. Additionally, the license holder failed to ensure that the license holder and program staff did not use funds of the client by the program to purchase items for which the facility is already receiving public or private payments.

On multiple dates, a staff person (P) made purchases such as spices, kitchen cookware, and food for all clients and for the facility using client’s money.

Corrective Action Ordered: Immediately and continuously, the license holder must ensure to comply with this statute.

If you choose to exercise this right, your request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this letter.
