CORRECTION ORDER
An investigation of Catholic Charities Mother Theresa Home/ Catholic Charities In Home Program was completed regarding report number 202500901, which alleged violation of Minnesota Statutes, section 626.557, and Minnesota Statutes, Chapter 245D. This maltreatment allegation concerned neglect. The disposition of the investigation report is substantiated. One violation was noted.
VIOLATIONS AND CORRECTION ORDERS
The following violation of state and (or) federal laws and rules was observed. Corrective action for the violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violations within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07.
Citation: Minnesota Statutes, section 245D.07 subdivision 1a.
Violation: For one person (P1) whose record was reviewed, the license holder did not provide services in response to the person’s identified needs as required. P1’s identified needs as specified in his/her support plan and support plan addendum, included no unsupervised time in the community, door alarms on the front and back door, and having a 1:1 staff person from 7 a.m. to 8 p.m.
On January 31, 2025, P1 left the facility without staff persons knowledge or supervision and was located in the community unsupervised. In addition, during the course of the investigation it was learned that on the day of the incident, one or more of the facility’s door alarms was not working and that the VA was not provided with 1:1 supervision from 7 to 8:43 a.m.
Corrective Action Ordered: Correct immediately and maintain compliance with this licensing requirement on a continuing basis throughout the program.
If you believe any of the citations are in error, you may ask the Commissioner of Human Services to reconsider the parts of the correction order that you believe to be in error. If you choose to exercise this right, your request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this letter.
