June 17, 2025 Sarah Lovejoy, Authorized Agent Kootasca Head Start Grand Rapids 822 Northeast 5th Avenue Grand Rapids, MN 55744 License Number 1025915 (Child Care) ORDER RESCINDING ORDER TO PAY A FINE Dear Sarah Lovejoy: On June 3, 2025, the Department of Human Services (DHS) issued Kootasca Head Start Grand Rapids an Order to Pay a Fine (“Fine Order”) based on a background study violation. On June 3, 2025, DHS received Kootacsa Head Start Grand Rapids appeal of the Fine Order. Following the issuance of the Fine Order, DHS received additional information relevant to the background study violation. After reviewing this information, DHS has determined that there is insufficient evidence to support the violation for which the fine was issued. As a result, the June 3, 2025, Order to Pay a Fine and the background study violation therein is hereby rescinded. This Order supersedes the Order dated June 3, 2025, and an appeal of that original Order is no longer applicable.
