CORRECTION ORDER
Dear Fartun Wardere: On June 2, 2025, the Department of Human Services became aware of licensing violations regarding compliance with state and federal laws and rules governing the provision of substance abuse treatment under Minnesota Statutes, chapter 245G for Serenity Recovery LLC, located at 1821 University Avenue W, Suite 298, Saint Paul, MN 55104. As a result, a Correction Order is being issued.
LICENSING VIOLATIONS
DHS determined that your program failed to follow licensing rules and statutes, as described below. Policies, Practices, and Procedures
1. Violation: The license holder failed to notify the Commissioner before the program changed locations. DHS licensing conducted an on-site visit on June 2, 2025, and noted that the license holder was not located in their license location, 1821 University Avenue W, Suite 281. On May 1, 2025, the license holder changed the location of the licensed program to an unlicensed location at 1821 University Avenue W, Suite 298, Saint Paul, MN 55104.
Statute Violated: Minnesota Statutes, sections 245A.04, subdivision 7, paragraphs (c) and (d), and 245G.03, subdivision 3. Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure licensing requirements are met.

If you fail to correct the violation(s) specified in the Correction Order within the prescribed time lines the Commissioner may issue an Order of Conditional License or may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07.
