CORRECTION ORDER
Dear Ali Kofiro:
On July 21, 2025, a licensing review of Care Connection Services LLC, located at 2626 East 82nd Street Bloomington, Minnesota, was conducted to determine compliance with state and federal laws and rules governing the provision of home and community-based services to persons with disabilities and age 65 and older under Minnesota Statutes, Chapter 245D. As a result of this licensing review a Correction Order is being issued.
A. Reason for Correction Order
Pursuant to Minnesota Statutes, section 245A.06, if the Commissioner of the Department of Human Services (DHS) finds that the license holder has failed to comply with an applicable law or rule and this failure does not imminently endanger the health, safety, or rights of the persons served by the program, the Commissioner may issue a Correction Order to the license holder.
The following violation(s) of state or federal laws and rules were determined as a result of the licensing review. Corrective action for each violation is required by Minnesota Statutes, section 245A.06 and is hereby ordered by the Commissioner of Human Services.
1. Citation: Minnesota Statutes 245A.65, subdivision 1, paragraph (b).
Violation: For one of five persons whose records were reviewed (P5), the license holder did not enforce written policies and procedures related to suspected or alleged maltreatment as required.
In December 2023, the license holder completed an internal review of reported incident(s) of suspected maltreatment and/or licensing violations involving P5. The license holder did not include in this review an evaluation of whether policies and procedures were followed, whether policies and procedures were adequate, whether there is a need for additional staff training, and where the reported event was similar to past events with P5 or the services involved.
Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subpart.
2. Citation: Minnesota Statutes, section 245A.65, subdivision 2.
Violation: For two persons whose records were reviewed (P1 and P2), the license holder did not develop and review an individual abuse prevention plan (IAPP) as required.
245A.02, subdivision 2b defines “annual” or “annually” to mean prior to or within the same month of the subsequent calendar year.
a. The license holder did not develop an IAPP for P1 that included:
· the statements of the specific measures to be taken to minimize the risk of abuse to P1 and other vulnerable adults; and
· the specific actions the license holder would take to minimize the risk of abuse to P1 within the scope of licensed services.
The license holder developed P1’s IAPP which assessed that P1 was vulnerable to sexual abuse and physical abuse. The license holder did not include a statement of measures, that included specific actions, that would be taken to minimize the risk of abuse to P1 in these areas.
b. The license holder did not document the review of P2’s IAPP at least annually in 2025. The license holder documented the review of P2’s IAPP in May 2024, and July 2025.
Corrective Action Ordered: Within 30 days of receiving this order, you must review and revise P1’s IAPP to include the information identified above, review the updated IAPP with P1’s interdisciplinary team, and revise the plan to reflect the results of this review. Additionally, you must provide P1’s staff with training on the updated IAPP and maintain documentation of this in accordance with Minnesota Statues 245D.095. On an ongoing basis, you must maintain compliance as required in this subdivision.
3. Citation: Minnesota Statutes, section 245D.04, subdivision 1.
Violation: For one person whose record was reviewed (P2), the license holder did not provide a written notice that identified the service recipient rights annually as required.
The license holder did not provide P2 with a written notice that identified the service recipient rights, and an explanation of those rights annually in 2025. The license holder provided P2 with the service recipient right in May 2024, and July 2025.
Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision.
4. Citation: Minnesota Statutes, section 245D.05, subdivision 1, paragraph (b).
Violation: For one person whose record was reviewed (P1), the license holder did not maintain documentation of how health needs would be met as required.
The license holder was assigned responsibility for meeting P1’s health needs in their support plan addendum. The license holder did not maintain documentation on how P1’s health needs would be met that included a description of the procedures the license holder would follow in order to use medical equipment safely and correctly according to written instructions from a licensed health professional.
Corrective Action Ordered: Within 30 days of receiving this order, you must maintain documentation on how P1’s health needs will be met, including the information identified above. Additionally, you must provide staff with training on P1’s updated health needs, including how to safely and correctly use P1’s medical equipment as instructed by a licensed professional, and maintain documentation of this in accordance with Minnesota Statues 245D.095. On an ongoing basis, you must maintain compliance as required in this subdivision.
5. Citation: Minnesota Statutes, section 245D.07, subdivision 1.
Violation: For one person whose record was reviewed (P4), the license holder did not ensure provision of services comply with federal waiver plans as required.
The license holder was assigned to provided P4 with individualized home supports with family training. The license holder hired and paid P4’s siblings to provide this service to P4. According to the community based services manual (CBSM), relatives cannot be paid to provide this service.
Corrective Action Ordered: Immediately upon receiving this order you must comply with the requirements of the federal waiver plan for the service of individualized home supports with family training, as instructed in the CBSM. You must ensure the service individualized home supports with family training is not provided to P4 by their relatives. On an ongoing basis, you must maintain compliance as required in this subdivision.
6. Citation: Minnesota Statutes, section 245D.07, subdivision 3.
Violation: For one person whose record was reviewed (P1), the license holder did not provide written reports as required.
The license holder maintained in P1’s support plan addendum that P1, P1’s case manager, or the team requested semi-annual written reports regarding P1’s progress or status. At the time of the licensing review, the license holder had not provided any written reports to P1 or their support team.
Corrective Action Ordered: Within 30 days of receiving this order, you must provide P1 and their support team with the information identified above. On an ongoing basis, you must maintain compliance as required in this subdivision.
7. Citation: Minnesota Statutes, section 245D.095, subdivision 3.
Violation: For three persons whose records were reviewed (P1, P3, and P4), the license holder did not maintain service recipient records as required.
The license holder did not maintain progress or daily log notes recorded by the program for P1, P3, and P4.
Corrective Action Ordered: Within 30 days of receiving this order, you must maintain daily log notes recorded by the program for each person to whom you are providing direct support services. On an ongoing basis, you must maintain compliance as required in this subdivision.
8. Citation: Minnesota Rules, chapter 9544.0030, subpart 1.
Violation: For two persons whose records were reviewed (P1 and P2), the license holder did not evaluate positive support strategies as required.
The license holder did not evaluate the identified positive support strategies with P1 and P2 at least every six months. · The license holder evaluated P1’s positive support strategies in May 2023; May 2024; and May 2025.
· The license holder evaluated P2’s positive support strategies in May 2024; June 2024; and July 2025.
Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subpart.
9. Citation: Minnesota Statute 245D.095, subdivision 5, paragraph (a).
Violation: For two of seven staff persons whose records were reviewed (SP1 and SP2), the license holder did not maintain personnel records as requires.
a. The license holder provided SP1 with orientation to individual service recipient needs in November 2022, but did not maintain the number of hours per subject area in SP1’s personnel record.
b. The license holder provided SP with orientation to individual service recipient needs and topics determined necessary in September 2024, but did not maintain the number of hours per subject area in SP2’s personnel record.
Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subpart.
If you fail to correct the violations specified in the Correction Order within the prescribed time lines the Commissioner may issue an Order of Conditional License or may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07.
B. Right to Request Reconsideration
If you believe any of the citations are in error, you have the right to request that the Commissioner of Human Services reconsider the parts of the Correction Order that you believe to be in error. The request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this report. Your request for reconsideration must be sent to:

Please note that a request for reconsideration does not stay any provisions or requirements of the Correction Order. The Commissioner’s disposition of a request for reconsideration is final and not subject to appeal under Minnesota Statutes, chapter 14.
