August 28, 2025 Aaron Fisk 157 Roosevelt Road Ste 200 St. Cloud, MN 56301 RE: Report No. 202406411 Catholic Charities Mother Theresa Home License No. 1070419/1070417 Dear Aaron Fisk: CORRECTION ORDER An investigation of Catholic Charities Mother Theresa Home was completed regarding report number 202406411 which alleged violation of Minnesota Statutes, section 626.557, and Minnesota Statutes, Chapter 245D. This maltreatment allegation concerned neglect. The disposition of the investigation report is inconclusive. Two violations were noted. VIOLATIONS AND CORRECTION ORDERS The following violations of state and (or) federal laws and rules were observed. Corrective action for each violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violations within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07. 1. Citation: Minnesota Statutes, section 245D.04, subdivision 3. Violation: The license holder did not ensure the protection and exercise of the facility clients’ rights to receive services in a clean and safe environment and to have use of and free access to common areas in the facility. On August 1, 2024, the facility had fecal matter on walls in various areas and torn disposable undergarments in a bedroom and at the top of a stairway. In addition, two bathrooms which were used by facility clients were locked so did not allow for clients to freely access the bathrooms. Corrective Action Ordered: Immediately and continuously, the license holder must ensure to comply with this statute. 2. Citation: Minnesota Statues 245D.06, subdivision 2. Violation: The license holder did not meet the protection standards as required. On August 1, 2025, staff persons stated they could not clean the feces on the walls of the facility because there were no disposable gloves available. Corrective Action Ordered: Immediately and continuously, the license holder must ensure to comply with this statute. If you believe any of the citations are in error, you may ask the Commissioner of Human Services to reconsider the parts of the correction order that you believe to be in error. If you choose to exercise this right, your request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this letter.
