License Holder agrees to pay $150 of the ordered fine. 2. DCYF agrees to stay the remainder of the fine for a period of two years. 3. License Holder agrees the three background study violations are conclusive. 4. License Holder agrees that within 15 days of final execution of this Agreement, the program will submit written documentation to its DCYF licensor detailing how the program will maintain compliance with Background Study Requirements.
