Citation: Minnesota Statutes 245D.09, subdivision 4a, paragraph (c)

Violation: The license holder did not ensure that before having unsupervised direct contact with a person served by the program, the staff person must review and receive instruction on each person's support plan or support plan addendum, individual abuse prevention plan, to achieve and demonstrate an understanding of the person as a unique individual, and how to implement those plans.

For one staff (S1) record reviewed, S1 began working at the facility on July 2, 2024, and received training on a client’s (C1) support plan addendum that same day.
