November 3, 2025 Timothy Osmondson, Authorized Agent Jobs House of Prosperity 7581 9th Street North, Suite 100 Oakdale, MN 55128 RE: Report No. 202506127 Jobs House of Prosperity License No. 1103884 Dear Timothy Osmondson: CORRECTION ORDER An investigation of Jobs House of Prosperity was completed regarding report number 202506127, which alleged violation of Minnesota Statutes, section 626.557, and Minnesota Statutes, Chapter 245D. This maltreatment allegation concerned neglect and physical abuse. The disposition of the investigation report is substantiated neglect and physical abuse. Three violations were noted. VIOLATIONS AND CORRECTION ORDERS The following violations of state and (or) federal laws and rules was observed. Corrective action for each violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violations within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07. 1. Citation: Minnesota Statutes, section 245D.07, subdivision 1a, paragraph (a) Violation: For one client whose record was reviewed (C1), the license holder did not provide services in response to the person’s identified needs, interests, preferences, and desired outcomes as specified in the support plan and the support plan addendum as required. Information was provided that on July 10, 2025, a staff person (S1) blocked C1’s exit from the room when C1 was “upset,” which was one of C1’s coping skills (to take a break or a walk). S1 also did not encourage C1’s coping skills or provide “Reminders and verbal cues … to teach [C1] to remember to use [his/her] coping skills independently.” Also on July 10, 2025, staff persons (S1 and S2) left C1 unsupervised in his/her room for an undetermined amount of time, which was inconsistent with C1’s supervision plan. Later that same day, C1 was unsupervised for an unknown amount of time in the hotel lobby with a hotel staff person. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. 2. Citation: Minnesota Statutes 245D.061, subdivision 8 Violation: The license holder did not report within five working days the use of an emergency use of manual restraint to the Department of Human Services and the Office of the Ombudsman for Mental Health and Developmental Disabilities. Information was provided that on July 10, 2025, S1 used an unspecified “restraint” on C1, which lasted about 15 seconds. The incident was not reported. Corrective Action Ordered: Immediately report the, July 10, 2025, use of an emergency use of manual restraint to the Department of Human Services and the Office of the Ombudsman for Mental Health and Developmental Disabilities, and on an ongoing basis you must maintain compliance as required in this subdivision. 3. Citation: Minnesota Statutes, section 245D.06, subdivision 5 Violation: For one client whose record was reviewed (C1), the license holder used a prohibited procedure as a behavioral or therapeutic program to reduce or eliminate behavior. Information was provided that on July 10, 2025, the license holder implemented prohibited procedures to reduce or eliminate C1’s behaviors, including staff (S1) straddling C1’s back on the floor with S1’s arms wrapped around C1’s neck. Corrective Action Ordered: On an ongoing basis you must maintain compliance as required in this subdivision. If you believe any of the citations are in error, you may ask the Commissioner of Human Services to reconsider the parts of the correction order that you believe to be in error. If you choose to exercise this
right, your request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this letter.
