November 4, 2025 Marvin Ladelle Roseman, Authorized Agent 7581 9th Street North Suite 100 Oakdale, MN 55128 RE: Report No. 202504860 Jobs House of Prosperity License No. 1124257/1103884 Dear Marvin Ladelle Roseman: CORRECTION ORDER An investigation of Jobs House of Prosperity was completed regarding report number 202504860 which alleged violation of Minnesota Statutes, section 626.557, and Minnesota Statutes, Chapter 245D. This maltreatment allegation concerned physical abuse. The disposition of the investigation report is inconclusive. One violation was noted. VIOLATIONS AND CORRECTION ORDERS The following violation of state and (or) federal laws and rules was observed. Corrective action for the violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violations within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07. Citation: Minnesota Statutes, section 245A.04, subdivision 5, paragraph (a), clauses (2) and (4), states that the license holder must provide access to documents and records, including staff personnel records of current and former staff, in a manner that allows for review by the commissioner. Violation: As part of a maltreatment investigation, a DHS investigator requested the license holder to submit documents and records relating to the alleged maltreatment in obtaining and reviewing information that was needed to complete the investigation. Attempts were made on August 27, October 8 and 16, 2025, to administrative staff by the DHS investigator to obtain information; however, the following requested documents and records were not provided to the DHS investigator: · Death or Serious Injury Report for a resident (R) who received treatment by a physician on April 14, 2025, for an injury that occurred while receiving care and services from the license holder; · Behavior Intervention Reporting Forms involving multiple incidents in which staff persons used emergency use of manual restraints (EUMR) when responding to the R’s behaviors; · The license holder’s polices that included Reporting of Maltreatment of Vulnerable Adults, EMUR Policies, Client/Service Recipient’s Rights, and policies relating to behavior management; and · Records of staff time sheets and/or clarification of this information for April 14 and 15, May 1 and 19, June 13, and September 19, 2025. Corrective Action Ordered: Immediately and on an ongoing basis you must maintain compliance with the section. If you believe any of the citations are in error, you may ask the Commissioner of Human Services to reconsider the parts of the correction order that you believe to be in error. If you choose to exercise this right, your request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this letter.
