CORRECTION ORDER
Dear Jeannie: On November 6, 2025, a certification review of Heartland Ranch, Inc., located at 185 Highway 9 NE Benson, MN 56215, was conducted to determine compliance. This review was conducted to determine compliance with state and federal laws and rules governing the provision of The Family First Prevention Services Act under Minnesota Statues 245A.25. As a result of this certification review a Correction Order is being issued. LICENSING VIOLATIONS
DHS determined that your program did not follow licensing rules and statutes, as described below. Personnel
1. Violation: Three of three personnel files reviewed for requirements of orientation training did not meet requirements. There was no documentation to support that the staff received training on the following topics prior to having direct contact with a youth:
a. Concepts of trauma informed care and how to provide services to each youth according to those concepts (personnel files numbered 4, 5, and 6); and
b. Impacts of each youth’s culture, race, gender, and sexual orientation on the youth’s behavioral health and traumatic experiences (personnel files numbered 4, 5, and 6).
Statute Violated: Minnesota Statutes, section 245A.25, subdivision 3, paragraph (h). Repeat Violation: In a Correction Order that DHS issued on June 7, 2023, you were previously found in violation of this same statute. Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that orientation training meets all applicable requirements. Within 30 days receipt of order, submit documentation of a staff orientation plan that demonstrates compliance with all requirements. 2. Violation: One of six personnel files reviewed for requirements of ongoing training did not meet requirements. There was not documentation to support that staff received training on the impacts of each youth’s gender and sexual orientation on the youth’s behavioral health and traumatic experiences for the calendar year 2025 (personnel file numbered 6).
Statute Violated: Minnesota Statutes, section 245A.25, subdivision 3, paragraph (h). Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that ongoing training meets all applicable requirements.

If you fail to correct the violation(s) specified in the Correction Order within the prescribed time lines the Commissioner may issue an Order of Conditional License or may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07.
