December 9, 2025 Stephanie Mox, Authorized Agent 2750 Blue Water Rd, STE 275 Eagan, Minnesota 55121 RE: Program Name: Companion Linc LLC License Number: 1080893 (245D – HCBS), Report No. 202501305 CORRECTION ORDER RECONSIDERATION DECISION Dear Stephanie Mox: The Minnesota Department of Human Services (DHS), Division of Licensing, received your request for reconsideration, dated May 15, 2025, regarding Citation 2 in the Correction Order dated April 30, 2025. After an independent review of the record, the Commissioner has determined that there is sufficient evidence to rescind Citation 2. Because you requested reconsideration only of Citation 2, other citations are final and not within the scope of this review. Citation 2 Violation You were issued a correction order for failure to meet the requirements for safekeeping of funds as required for one person whose record was reviewed. You assisted with the safekeeping of funds, however, you did not initially demonstrate that you documented and implemented the preferences of the person’s legal representative and case manager for frequency of receiving a statement that itemizes receipts and disbursements of funds or other property. Applicable Law When assisting a person served by the program with safekeeping of funds, the license holder must obtain written authorization to do so from the person or the person’s legal representative and case manager which must be renewed annually. At the time initial authorization is obtained, the license holder must survey, document, and implement the preferences of the person or the person's legal representative and the case manager for frequency of receiving a statement that itemizes receipts and disbursements of funds or other property. The license holder must document changes to these preferences when they are requested. Minnesota Statutes, § 245D.06, subdivision 4, paragraph (a). Your Response You have requested reconsideration of Citation 2 because you state that your program completes Annual Consent forms which contain the required consents and authorizations for all individuals supported with safekeeping of funds. In your reconsideration request, you submitted the Annual Consent and Releases for Services for the person in question, including the funds and property authorization to assist in safekeeping funds and the meeting and report signature page. Reconsideration Decision On reconsideration, you provided the Annual Consent and Releases for Services forms referenced above as well as a meeting and report signature page confirming that the person and their team approved of the annual report and review. The Annual Consent form states the frequency of receiving a statement that itemizes receipts and disbursements of funds or other property. After reviewing the funds and property form and documentation provided, you have demonstrated that your program is in compliance with statutory requirements. Disposition The Commissioner has reviewed the relevant laws and the information you submitted in response to the Correction Order. Citation 2 is rescinded. This is a final agency decision. Sincerely,
