December 8, 2025 Jill Cihlar 724 Central Ave NE Minneapolis, MN 55414 RE: Report No. 202506785 Pinnacle 19th Ave/Pinnacle Services Inc License No. 1109259/1069733 Dear Jill Cihlar: CORRECTION ORDER An investigation of Pinnacle 19th Ave/Pinnacle Services Inc was completed regarding report number 202506785 which alleged violation of Minnesota Statutes, section 626.557, and Minnesota Statutes, Chapter 245D. This maltreatment allegation concerned financial exploitation. The disposition of the investigation report is substantiated. One violation was noted. VIOLATION AND CORRECTION ORDER The following violation of state and (or) federal laws and rules was observed. Corrective action for the violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violation within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07. Citation: Minnesota Statutes, section 245 D.10, subdivision 1. Violation: The license holder did not enforce policies and procedures as required. The license holder did not ensure that financial documents/statements were audited according to the facility policies and procedures regarding Safeguarding Funds of Individuals Served and did not ensure implementation of facility program policies and procedures by program staff. Corrective Action Ordered: On an ongoing basis, you must comply with Minnesota Statutes, section 245D.10, subdivision 1 requirements related to establishing, enforcing, and maintaining policies and procedures as required. If you believe any the citation is in error, you may ask the Commissioner of Human Services to reconsider the parts of the correction order that you believe to be in error. If you choose to exercise this right, your request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this letter.
