December 11, 2025 Stephanie Kalbach, Authorized Agent REM River Bluffs, Inc. 6600 France Avenue South, Suite 350 Edina, MN 55435 RE: Report No. 202507549 REM River Bluffs, Inc.-Opal License No. 1071922/1071879 Dear Stephanie Kalbach: CORRECTION ORDER An investigation of REM River Bluffs, Inc.-Opal was completed regarding report number 202507549, which alleged violation of Minnesota Statutes, section 626.557, and Minnesota Statutes, Chapter 245D. This maltreatment allegation concerned financial exploitation. The disposition of the investigation report is substantiated. Two violations were noted. VIOLATIONS AND CORRECTION ORDERS The following violations of state and (or) federal laws and rules was observed. Corrective action for each violation is required by Minnesota Statutes, section 245A.06, and is hereby ordered by the Commissioner of Human Services. Failure to correct the violations within the prescribed amount of time may result in fines and/or action against your license, as provided for in Minnesota Statutes, sections 245A.06 and 245A.07. 1. Citation: 245D.095, subdivision 5, paragraph (a), clause (2) Violation: For one of three staff persons whose record was reviewed (S1), the license holder did not document training. The license holder did not document S1’s training on a client’s (C1) support plan, including Risk Assessment Detail. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required with this subdivision. 2. Citation: Minnesota Statutes, section 245D.07, subdivision 1a, paragraph (a) Violation: For one client whose record was reviewed (C1), the license holder did not provide services in response to the person’s identified needs, interests, preferences, and desired outcomes as specified in the support plan and the support plan addendum as required. On July 20, 2025, a staff person (S1) left C1 unsupervised in a car for an unknown amount while S1 went inside at least two different stores. Staff should always supervise C1 when in the community. Corrective Action Ordered: On an ongoing basis, you must maintain compliance as required in this subdivision. If you believe any of the citations are in error, you may ask the Commissioner of Human Services to reconsider the parts of the correction order that you believe to be in error. If you choose to exercise this right, your request for reconsideration must be in writing and received by the Commissioner within 20 calendar days after receipt of this letter.
