CORRECTION ORDER

Dear Jill:

On November 17, 18, and 19, 2025, Department of Human Services (DHS) licensors conducted a licensing review at your facility, Integrations Wellness and Recovery Center LLC located at 1135 Hwy 7 W, Hutchinson, MN 55350. This review was conducted to determine compliance with state and federal laws and rules governing the provision of Substance Use Disorder Treatment under Minnesota Statute 245G. As a result, DHS is issuing this order which requires you to take the correction action as described under each violation. Details of our findings are provided below. Our next steps and your options are also detailed.

LICENSING VIOLATIONS

DHS determined that your program failed to follow licensing rules and statutes, as described below.

Policies, Procedures, and Practices

1. Violation: The license holder did not meet requirements for receiving public funding reimbursement from the commissioner for services provide for individuals with co-occurring disorders in accordance with Minnesota Statutes, section 254B.0507, subdivision 6. There was no documentation of the following:

a. That a mental health diagnostic assessment was completed within 10 days of admission (client file numbered 2); and

b. 8 hours of co-occurring training for:

1) Calendar year 2023 (personnel file numbered 4); and

2) Calendar year 2024 (personnel file numbered 4).

Statute Violated: Minnesota Statues, section 245A.191, paragraph (a).

Corrective Action Required: Immediately, and on an ongoing basis, the license holder must ensure and document that services provided meet the requirements for payment from the behavioral health fund. The noncompliance identified above may result in nonpayment of claims submitted by the license holder for public program reimbursement; recovery of payments made for the services; disenrollment in the public payment program; or other administrative, civil, or criminal penalties provided by law.

2. Violation: The license holder did not meet requirements governing program abuse prevention plans. The program abuse prevention plan was not reviewed annually by the governing body or governing body’s delegated representative for calendar year 2023.

Statute Violated: Minnesota Statutes, section 245A.65, subdivision 2, paragraph (a), clause (5).

Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that program abuse prevention plans meet all applicable requirements.

3. Violation: The license holder did not meet requirements governing vulnerable adult reporting and internal reviews. There was no documentation of the following alleged maltreatment reported on August 29, 2025:

a. An internal review completed within 30 calendar days; and

b. A written notice to the mandated reporter within two working days stating whether the facility has reported the incident to the common entry point.

Statute Violated: Minnesota Statutes, section 245A.65, subdivision 1, paragraph (b), clause (1).

Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that vulnerable adult reporting internal reviews meet all applicable requirements.

4. Violation: The license holder did not meet requirements governing the plan for transfer of clients and records upon closure. There was no documentation that a controlling individual reviewed the plan in calendar year 2024.

Statute Violated: Minnesota Statues, section 245A.04, subdivision 15a, paragraph (a).

Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that transfer of clients and records meets all applicable requirements.

Personnel Files

5. Violation: One personnel file was reviewed for staff qualifications for individuals with temporary permit (personnel file numbered 5) and did not meet requirements. The supervising licensed alcohol and drug counselor did not document the amount and type of supervision required on a weekly basis for the months of May, June, July, August, September, and October 2025.

Statute Violated: Minnesota Statutes, section 245G.11, subdivision 11, paragraph (a).

Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that staff qualifications meet all applicable requirements.

6. Violation: Three of three personnel files reviewed for requirements governing annual and additional trainings did not meet requirements. There was no documentation of the following:

a. Mandatory reporting as specified in Minnesota Statutes, sections 245A.65, 626.557, 626.5572, and chapter 260E, including specific training covering the license holder’s policies for obtaining a release of client information for:

1) Calendar year 2023 (personnel file numbered 4);

2) Calendar year 2024 completed late (personnel file numbered 7); and

3) Calendar year 2025 completed late (personnel files numbered 4 and 7).

b. Release of Information for calendar year 2023 (personnel file numbered 4).

c. Program abuse prevention plan for:

1) Calendar year 2023 (personnel file numbered 4).

2) Calendar year 2024 completed late (personnel file numbered 7); and

3) Calendar year 2025 (personnel file numbered 4 and 7).

d. All internal policies and procedures related to the prevention and reporting of maltreatment of vulnerable adults for:

1) Calendar year 2023 (personnel file numbered 4);

2) Calendar year 2024 (personnel file numbered 4 and 7); and

3) Calendar year 2025 (personnel file numbered 7).

e. HIV minimum standards:

1) Calendar year 2023 (personnel file numbered 4);

2) Calendar year 2024 completed late (personnel file numbered 7); and

3) Calendar year 2025 (personnel files numbered 4 and 7).

Statute Violated: Minnesota Statutes, sections 245A.19, paragraph (b) and 245G.13, subdivision 2, paragraph (c) and (d).

Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that staff trainings meet all applicable requirements. Within 30 days of receipt of this order, submit documentation that the staff above have received required trainings.

7. Violation: One of two personnel files reviewed for requirements governing additional training did not meet requirements. There was no documentation of 12 hours of training in co-occurring disorders (personnel file numbered 5).

Statute Violated: Minnesota Statutes, section 245G.13, subdivision 2, paragraph (f)

Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that additional training meets all applicable requirements.

8. Violation: Four of four personnel files reviewed for requirements governing personnel file contents did not meet requirements. There was no documentation of the following:

a. Application signed by the staff member containing the staff members qualifications for employment (personnel files numbered 4, 5, 6, and 7); and

b. Annual written job performance review:

1) For calendar year 2023 and 2024 (personnel file numbered 4); and

2) For calendar year 2024 and 2025 (personnel file numbered 7).

Statute Violated: Minnesota Statutes, section 245G.13, subdivisions 1 and 3.

Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that personnel file contents meet all applicable requirements.

Client Files

9. Violation: Two of four client files reviewed for requirements governing initial service plans (ISP) did not meet requirements in the following ways

a. The ISP was not client specific (client files numbered 3 and 4); and

b. The ISP did not identify treatment needs to be addressed during the time between the day of service initiation and development of the individual treatment plan (client file numbered 4).

Statute Violated: Minnesota Statutes, section 245G.04, subdivision 1.

Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that initial service plans meet all applicable requirements.

10. Violation: One of four client files (client file numbered 4) reviewed for requirements governing vulnerable adult determination did not meet requirements. The vulnerable adult assessment and determination was not completed within 24 hours of the day of service initiation.

Statute Violated: Minnesota Statutes, section 245G.04, subdivision 2, paragraph (a).

Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that vulnerable adult determination meet all applicable requirements.

11. Violation: Two of two files reviewed for requirements governing individual abuse prevention plans (IAPP) did not meet requirements in the following ways:

a. IAPP did not include an assessment of the person’s susceptibility to abuse (client files numbered 1 and 2); and

b. IAPP did not address what specific measures the program will take to minimize the risk of abuse (client file numbered 1).

Statute Violated: Minnesota Statutes, sections 245A.65, subdivision 2, paragraph (b), 245G.04, subdivision 2, paragraph (b), and 245G.21, subdivision 6.

Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure individual abuse prevention plans meet all applicable requirements. Within 30 days of receipt of this order, submit one individual abuse prevention plan that meets requirements.

12. Violation: Three of four files reviewed for the requirements governing comprehensive assessments (CA) did not meet the requirements in the following ways:

a. CA was not completed within 5 calendar days from the day of service initiation (client files numbered 1 and 2); and

b. Person-centered reason for delay and the planned completion date was not documented (client file numbered 1); and

c. CA did not include the following information:

1) Substance use history including:

i. Amount (client files numbered 2 and 4); and

ii. Route of administration (client file numbered 4);

2) Client’s resources (client file numbered 2); and

3) Important developmental incidents in the client’s life (client files numbered 1, 2, and 4).

Statute Violated: Minnesota Statutes, sections 245G.05, subdivisions 1, paragraphs (a) and (c) and 3.

Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure comprehensive assessments meet all applicable requirements.

13. Violation: Three of four client files reviewed for requirements governing individual treatment plans (ITP) did not meet requirements in the following ways:

a. ITP was not completed within 10 days from the date of service initiation (client file numbered 1);

b. ITP was not updated based on new information gathered about the client’s condition (client files numbered 3 and 4);

c. ITP was not based on the client’s comprehensive assessment (client file numbered 4);

d. ITP was not person-centered (client file numbered 3);

e. ITP did not include a treatment strategy (client file numbered 1); and

f. ITP did not identify resources to refer the client to when the client’s needs will be addressed concurrently by another provider (client file numbered 3).

Statute Violated: Minnesota Statutes, sections 245G.06, subdivisions 1 and 1a, paragraph (a), clauses (1) through (3), and (6).

Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure individual treatment plans meet all applicable requirements.

14. Violation: Four of four client files reviewed for requirements governing treatment plan reviews (TPR) did not meet requirements. There was no documentation of:

a. The span of time covered by the review (client files 1, 2, 3, and 4);

b. Whether the identified methods continue to be effective (client files numbered 1, 2, 3, and 4);

c. Toxicology results (client files numbered 3 and 4); and

d. Collaboration with continuing mental health providers (client file numbered 3).

Statute Violated: Minnesota Statutes, sections 245G.06, subdivision 3, clauses (1) and (2), and 245G.20, clause (6).

Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure treatment plan reviews meet all applicable requirements. Within 30 days of receipt of this order, submit a treatment plan review that demonstrates compliance.

15. Violation: One of two client files reviewed for requirements governing medication administration (client file numbered 1) did not meet requirements. There was no documentation of client’s use of medication including staff signatures with date and time October 17, 2025.

Statute Violated: Minnesota Statutes, section 245G.08, subdivision 5, paragraph (c), clause (7).

Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that medication administration meets all applicable requirements.

16. Violation: One of two client files reviewed for requirements governing service discharge summary (client file numbered 5) did not meet requirements. The discharge summary did not include client strengths and needs while participating in treatment.

Statute Violated: Minnesota Statutes, section 245G.06, subdivision 4, paragraph (b), clause (1).

Corrective Action Required: Immediately and on an ongoing basis, the license holder must ensure that discharge summaries meet all applicable requirements.

Written Response Required

If you fail to correct the violation(s) specified in the Correction Order within the prescribed time lines the Commissioner may issue an Order of Conditional License or may impose a fine and order other licensing sanctions pursuant to Minnesota Statutes, sections 245A.06 and 245A.07.

You have the right to request reconsideration of this order and the cited violations. Your request must:

· Be in writing

· List each violation you are challenging and identify what is inaccurate or incomplete about the information in the order

· Supply information that is accurate or more complete

· Be made before the deadlines provided below

If you are mailing your request, it must be received by DHS within 20 calendar days from when you received this order. If you do not meet this deadline, you lose your right to request reconsideration. The timeline to appeal began when you received this order. Please send it to:
